For just $4.25 per month, the entire family can receive valuable discounts on dental, vision and hearing costs through an easy-to-use savings card.

The plan is NOT insurance; however, it can be used in addition to many insurance plans. The Cooperative Healthy Savings program was designed to make healthcare more affordable through valuable discounts on medical costs for electric cooperative members. By presenting the card at the time of service, members will receive an instant discount. The card may be used as many times as needed. Members, their spouse and any legal dependents are all eligible to use it. The Cooperative Health Savings Card is accepted at thousands of providers nationwide with discounts that include:

Dental Care

• Members save 15% to 50% on everything from general dentistry and cleanings to root canals, crowns and orthodontia.

• More than 76,000 available dental practice locations nationwide.

• Save on routine dental services such as X-rays and fillings as well as on specialty care such as orthodontics and periodontics, where available.

• Save 10% to 60% on eyewear at more than 12,000 locations nationwide (includes most frames, lenses and specialty items.)

Once your application has been processed, you’ll receive two ready-to-use Cooperative Healthy Savings cards along with your membership kit that includes a list of local participating providers. Additional cards can be ordered through Cooperative Healthy Savings for $2.50 each. The Cooperative Healthy Savings Card may not be used in conjunction with the Co-op Connections Card.
